If your first few steps out of bed each morning feel like you're stepping on a nail, you may be dealing with plantar fasciitis, one of the most common causes of foot and heel pain I see in clinic. It's a condition that can slowly build over weeks or months and, if left untreated, can start to affect your gait as well as your knees, hips, and even your lower back. Let's break it down and understand a bit more about what plantar fasciitis is and what you can do about it.


What Is Plantar Fasciitis?


The plantar fascia is a thick band of connective tissue that runs along the bottom of your foot, connecting your heel bone to your toes. It acts as a shock absorber and supports the arch of your foot every time you stand, walk, or run.

Plantar fasciitis occurs when this band becomes irritated and inflamed, usually from repetitive stress, strain or overloading. While the name suggests inflammation, in many cases of chronic plantar fasciitis, the tissue actually is suffering from degenerative changes, not only inflammation. The result, whether the problem is chronic or acute, is pain, stiffness and/or reduced function at the bottom of the foot.


Common Symptoms

  • Sharp heel pain, often most noticeable with your first steps in the morning or after periods of rest and prolonged sitting.

  • Pain that may decrease somewhat with movement and stretching but can get worse after long periods of standing or walking.

  • Tenderness when pressing on the heel, where the plantar fascia connects onto the heel bone.

  • Stiffness in the arch of the foot or near the heel, especially after exercise (rather than during it).

  • In some cases, mild swelling around the heel.


Causes

Plantar fasciitis is generally the result of multiple factors and is an overuse type condition rather than the result of injury or any one single cause. Common contributing factors include:

  • Repetitive strain of the plantar fascia from running, prolonged standing, or walking on hard surfaces.

  • Tightness in the calf muscles and Achilles tendon. This can limit ankle flexibility and increase tension through the plantar fascia.

  • Footwear that lacks adequate support or cushioning that can help with shock absorption.

  • Sudden increases in activity, such as a new running programme or a change in job that requires more time on your feet than you are used to. Like any new activity, your feet may not be adapted to the sudden load increase required.

  • Excess body weight increases the load on the plantar fascia with every step. Over time, this stress can lead to tissue irritation and pain.

  • Abnormal foot mechanics, such as flat feet (pes planus), high arches (pes cavus), or overpronation change how the foot naturally moves and where pressure is applied along the foot when standing and walking. With flat feet, the plantar fascia is constantly stretched from the decreased arch height, both of these factprs will decrease the foot's ability to shock absorb during day to day activities. Those with high arches tend to have rigid feet and a general lack of foot flexibility. The high arch pulls the foot into a shortened position, placing additional tension on the plantar fascia.



Who Suffers From Plantar Fasciitis?

Plantar fasciitis is fairly common and is estimated to affect around 1 in 10 people at some point in their life. It's especially common in:

  • Runners and athletes, due to repetitive impact through the feet.

  • People with occupations requiring long hours of standing or walking, such as retail staff, teachers, nurses, and hospitality workers

  • Adults aged 40 to 60, as the fat pad on the heel naturally thins with age and provides less cushioning, increased pressure is applied to the plantar fascia which can lead to pain.

  • People who are overweight or have a higher BMI, which increases the load on the fascia

  • Pregnant women, especially those late in the 2nd and 3rd trimesters. Pregnancy leads to a combination of factors that can lead to plantar fasciitis. Hormonal changes loosen the joints and ligaments in the body which can cause a flattening of the arches in the feet. Weight gain and a change to your center of gravity can change the way you walk and where pressure is placed on your feet causing increased stress on the plantar fascia. Post-partum recovery from plantar fasciitis can still take weeks to months.

  • Anyone who has recently changed footwear, increased their activity level, or started spending significantly more time on their feet


How Chiropractic Care Can Help

Because plantar fasciitis is fundamentally a biomechanical problem, chiropractic care is well suited to addressing it, both at the site of pain and further up the chain.

  • Adjustments of the Foot and Ankle: A lack of joint mobility may had contributed to the developement of plantar fasciitis but may also be the result as your body compensates for pain, which further limits ankle movement and adding increased strain on the plantar fascia. Gentle adjustments and mobilisations to the ankles and feet, combined with soft tissue work to the calf and plantar fascia itself, can help improve joint mobility and reduce tension through the arch of the foot.

  • Adjustments of the Lower Back and Pelvis: When one foot hurts, we naturally change the way we walk and move to avoid putting pressure on it, either by shifting our weight, shortening our stride, or favouring the opposite leg. Over time, this altered gait can place uneven stress on the knees, hips, pelvis, and lower back. Many people with plantar fasciitis develop secondary tightness or mobility restrictions in the lower back and pelvis simply from weeks of compensating. Addressing these areas with chiropractic adjustments can help normalise your gait pattern, reduce compensatory pain, and support a more even, balanced recovery as the root of the problem is addressed.

  • Shockwave Therapy: Extracorporeal shockwave therapy (ESWT) is a non-invasive technique that uses focused acoustic pulses delivered to the affected area to stimulate blood flow and kickstart your body's natural healing response. ESWT is an effective and safe option for reducing pain and improving foot function and it is often recommended when symptoms are not fully resolving with rest and stretching alone. Some research suggesting it may even outperform corticosteroid injections in the medium term! Clients often experience mild to moderate discomfort during treatment with some people reporting mild lingering soreness up to 24 hours after treatment. Treatment intensity can be modified to patient tolerance so make sure you communicate with your treatment provider during your session.

  • Rehabilitation Exercises: Calf stretches, plantar fascia stretches, and foot strengthening exercises are tailored to your specific needs, helping to build resilience in the fascia and reduce your risk of recurrence.

  • Footwear and Load Management Advice: Practical guidance on supportive footwear and gradually adjusting your activity levels can make a meaningful difference alongside hands-on treatment.


Home Care for Plantar Fasciitis

Aside from or alongside professional treatment, there are several practical steps you can take at home to manage symptoms and support your recovery:

  • Stretching: Perform gentle stretches for your calf muscles, Achilles tendon, and the plantar fascia itself. Reducing soft tissue tension can reduce strain on the plantar fascia.

  • Self-Massage: Gently massage the arch of your foot using your hands, a massage ball, or a golf ball to help release muscle tension and improve local circulation.

  • Ice (For Acute Pain): Apply an ice pack for 10–15 minutes during the initial, acute phase of pain to help soothe severe discomfort. But avoid using ice long-term, as prolonged cold application can reduce localized blood flow and slow down the tissue healing process. A great trick is to roll your foot over a frozen water bottle as this allows you to get the benefits of a massage and ice therapy at the same time! For safety, avoid using a glass bottle for this.

  • Pain & Anti-Inflammatory Medication: Over-the-counter pain medication can help manage severe discomfort in the short term. Always consult a healthcare professional or pharmacist before use.

  • Rest & Activity Modification: Reduce activities that aggravate the pain. If you’ve recently increased your running mileage, scale back temporarily or switch to low-impact alternatives like swimming or cycling to keep active without overloading the heel.

  • Footwear Changes: Avoid prolonged standing and walking barefoot, especially on hard surfaces. Wear supportive shoes with good arch support and adequate heel cushioning both outdoors and indoors.

  • Night Bracing: Wearing a night splint holds the foot in a gentle, passive stretch overnight. This prevents the plantar fascia from tightening while you sleep, making those painful first steps in the morning a bit more manageable.


Plantar fasciitis can feel stubborn and unrelenting, but with the right combination of hands on care, targeted rehab, and treatments like shockwave therapy, most people see significant improvement. If you are waking up to that familiar stab of heel pain every morning, don't wait for it to become a chronic problem. The sooner it is addressed, the easier it typically is to resolve.
